放射性核素肝胆显像诊断先天性胆道闭锁的临床价值(1)-找论文网

［摘要］目的：回顾性评价99 mTcEHIDA肝胆显像诊断先天性胆道闭锁(BA)的临床价值。方法：应用SPECT仪器对59例临床上持续黄疸的新生儿和婴儿进行99 mTcEHIDA肝胆显像，并经手术病理诊断及随访确诊。结果：最终诊断为BA的34例患儿中,99 mTcEHIDA肝胆显像全部检出；非BA组25例中23例为真阴性,2例为假阳性,灵敏度为100%，特异性92%，准确度96.6％。结论：99 mTcEHIDA肝胆显像是一种无创、安全、有效的检查方法，有助于明确病因,预测治疗效果,对于BA的诊断有较高的临床价值。

　　Clinical Value Of Radionuclide Hepatobiliary Scintigraphy on Diagnosis of Congential Biliary Atresia

Abstract：Objective To retrospectively evaluate the diagnostic value of 99 mTcEHIDA hepatobiliary scintigraphy in congential biliary atresia(BA)．Methods Fortynine infants and neonates(34 infants with BA，the other 25 with non—BA)with consistent jaundice received 99 mTcEHIDAhepatobiliary scintigraphy with SPECT instrument，the results of all patients were proved by pathology and clinical followup results．Results 99 mTcEHIDA hepatobiliary scintigraphy diagnosed 34 infants and neonates with last diagnosis BA correctly；23 infants and neonates with last diagnosis nonBA correctly，but 2 neonates falsepositive in all nonBA 25 patients.The sensitivity,specificity and correct rate of this method were 100％,92% and 96.6%,respectively．Conclusion 99 mTcEHIDA hepatobiliary scintigraphy is an anoninvasive,safety，valuable examing method.It will help us definite the etilogy,predict the therapeutic efficacy,and have high clinical value in the diagnosis of BA．

　　先天性胆道闭锁(BA)是新生儿及婴儿持续性黄疸最常见的原因之一，早期诊断有重要的临床意义。本研究回顾分析了59例持续黄疸婴儿肝胆显像及临床资料，旨在进一步探讨核素肝胆显像对BA的诊断价值。

　　1 资料和方法
　　
　　1.1 临床资料 2000年3月至2006年5月在我科行肝胆显像的新生儿及婴儿共59例。其中男21例，女38例。年龄28 d~158 d，平均年龄62 d。其最后诊断通过手术后的病理证实,未接受手术者,则通过临床随访1 a,以患者是否存活和临床诊断证实。根据最后诊断分为:BA组34例,其中34例术后病理证实为BA,17例为肝内型,13例为肝外型,表现为胆管严重狭窄、缺如或纤维化,另外4例死亡诊断为BA;非BA组25例,其中新生儿肝炎综合征(NIS)8例,婴儿肝炎综合征(IHS)13例,胆管畸形3例,生理性黄疸1例。

　　1．2 检查方法患儿检查前4 h~6 h禁食，口服10%水合氯醛，平卧于r相机探头下，静脉注射99 mTcEHIDA（3 mCi~5 mCi），分别于5 min、10 min、20 min、30 min、45 min、60 min行肝胆显像。显像仪器为GE公司产Millennium VG双探头SPECT，低能高分辨准直器，能量（140 kV）。观察肝脏对显像剂吸收、摄取、排泄，胆囊显像情况及肠道中放射性核素出现时间。若1 h肠道未见放射性，分别于2 h、4 h、6 h直至24 h延迟显像。若肠道仍无放射性，口服苯巴比妥10 mg/kg/d，连续7 d~10 d，再次行肝胆显像；若仍未见肠道放射性显影，则诊断为BA［1］。

　　1．3 统计学分析根据诊断效能的评价方法［2］，计算灵敏度、特异度、准确性。
