乳胶凝集试验快速诊断轮状病毒的临床应用(1)-找论文网

［摘要］目的：简述轮状病毒快速诊断法的临床应用。方法：用轮状病毒的快速鉴定试剂盒对153例1岁～7岁患儿的大便标本，做玻片法乳胶凝集试验，成功进行了轮状病毒致小儿腹泻、胃肠炎的快速诊断。结果：轮状病毒致小儿腹泻的阳性检出率为46.25%(37/80),致小儿胃肠炎的阳性检出率为38.36%(28/73),总阳性率为42.48%(65/153)，皆于2 min内出结果，阳性结果中无发现交叉反应，亦符合临床诊断。结论：该快速诊断法具有操作简便、快速、准确、特异性强、阳性率高等特点，可以在基层和农村医疗单位推广应用，它为治疗轮状病毒致腹泻、胃肠炎的患儿提供了及时准确的客观依据。

　　The Clinical Application of the Rapid Diagnosis of Rotavirus

　　Abstract： Objective To research into the clinical application of the rapid diagnosis of Rotavirus .Methods A rapid diagnosis of fecal specimens take from the 153 case 17 years old children was successfully determined by the kit for rapid identification of Rotavirus cause of diarrhoea and gastroenteristis using the Rotavirus latex agglutination test. Results The positive detecting rates of diarrhoea and gastroenteristis patients by infection Rotavirus were 46.25%(37/80), 38.36(28/73). Total positive rate of the 153 cases was 42.48%(65/153).These results of test were obtained within two minutes .In the positive results , non of the positive cases showed cross agglutination phenomena , complete identified with clinical diagnosis .Conclusion The procedures involved in the test are simple , the diagnosis in fast ,and the result is high accurate and specific . This method is particularly suitable for the use in rural hospital.

　　自从1973年轮状病毒在人体被检测出来后，已被证明是造成婴儿与儿童腹泻性疾病的主要病因［1］。近10年来对轮状病毒的研究与流行病学的调查，已发现全世界因急性腹泻或胃肠炎而入院的婴儿与儿童病例中，有50%～60%是因此病毒感染所引起，在世界各地都有散发性的流行［2］。轮状病毒的快速检测，对轮状病毒所致腹泻或胃肠炎的患儿提供了及时准确的客观依据，对帮助临床上治疗有很重要意义。我们应用了Murex公司的乳胶凝集试验(The Murex Rotavirus Latex Agglutination Test)的快速鉴定法，对来我院就医的有腹泻或胃肠炎症状的患儿进行轮状病毒的快速诊断，现将结果报告如下。

　　1 资料与方法

　　1.1 资料

　　1.1.1 一般资料 2005年9月至2006年3月期间因腹泻症状来我院诊治的1岁～7岁患儿153例,其中男89例，女64例。

　　1.1.2 轮状病毒的快速诊断试剂盒含1瓶试验乳胶悬浮液(Test Latex Suspension),1瓶质控乳胶悬浮液(Control Latex Suspension),1瓶阳性质控抗原液(Positive Control Antigen),1瓶抽提液(Extraction Buffer),还有25个滴加标本和试剂反应用的纸板和小木棒，皆由法国Murex公司生产并提供。

　　1.1.3 检查标本患儿的大便标本，收集患儿标本时是用清洁的塑料瓶子装盛，试验中所需标本量约1 g左右。

　　1.2 方法按Murex公司的轮状病毒乳胶凝集试验的方法进行。由于试剂盒是放2 ℃～8 ℃保存，所以首先要把试剂盒放于室温18 ℃～25 ℃一段时间，约10 min。取约1 g患儿大便标本放于1支清洁玻璃试管内，再加入1 ml抽提液，充分摇匀，静置5 min，再充分摇匀，然后离心沉淀3 000 r/10 min，吸取上清液，如图所示滴加标本与试剂。其中Test Latex ，Control Latex各加1滴(约40 μl)，标本上清液约加40 μl，用试剂盒内供应的小木棒搅匀，再摇动2 min，然后观察结果。

　　1.3 结果判定方法 Test Latex与上清液混匀，出现一定数量的白色小凝集块，不随摇动而散开，同时Control Latex与上清液混匀无白色小凝集块出现，为阳性结果。Test Latex与上清液混匀，无凝块出现，摇动时混合液像白色牛奶状均匀，Control Latex与上清液混匀，无白色小凝集块出现，为阴性结果，若是Control Latex与上清液混合出现白色小凝集块，而Test Latex与上清液混合无白色凝集块出现，则表示是非特异反应(Nonspecific Reaction)或是标本取样不正确。若是Test Latex，Control Latex与上清液混匀均出现白色凝集，则要取标本重做，且要看：如果Test Latex与上清液混匀先出现白色凝集，而Control Latex与上清液混匀均后出现白色凝集，则结果判断为阳性，反之，则判断为阴性。
