Treatment of operative incision disunion of 25 diabetes patients with insulin composition
【Abstract】Objective To investigate the clinical efficiency of diabetes patients with operative incision disunion treated by insulin composition. Methods The insulin composition was composed of insulin, vitamin C, gentamycin and hypertonic glucose. 25 cases were treated with the composition whose incision were not healed by the routine method of change dressings.Results All cases were healed and the average time was 11.7 days.Conclusion The insulin composition is one of better way of treating operative incision disunion of diabetes patients.

糖尿病患者行手术治疗时易发生切口不愈合或延迟愈合，按照常规方法换药，疗效欠佳。我院从1998年3月～2006年3月，对25例糖尿病手术切口不愈合的患者采用胰岛合剂治疗，取得了良好效果。现总结如下。
1 资料与方法
1.1 一般资料
本组25例患者中男11例，女14例；年龄38～71岁，平均51.3岁；术后空腹血糖8.2～14.7mmol/L,平均8.9mmol/L。手术类别：股骨骨折切开复位内固定5例，胫骨骨折切开复位内固定11例，肱骨骨折切开复位内固定7例，尺骨骨折切开复位内固定2例。
1.2方法
胰岛素合剂组成：50%葡萄糖液20ml中加入普通胰岛素5U，维生素C 1g，庆大霉素8万U。切口不愈合判断标准：按照正常拆线时间，切口边缘苍白开裂，有分泌物流出或有坏死组织。
诊断为切口不愈合后，创面边缘用安尔碘消毒，有坏死组织应清除；予以生理盐水及胰岛素合剂液冲洗创面后，胰岛素合剂纱布湿敷创面，每天换药1～2次。其中部分病例在诊断为切口不愈合后因某些原因仍常规换药2～4周后，切口仍不能愈合，再改用胰岛素合剂换药。
