【摘要】 为了研究维吾尔族、汉族多发性骨髓瘤(MM)患者血清白介素6(IL-6)水平差异及检测的临床意义,采用双抗体夹心ELISA技术检测80例MM患者(33例维吾尔族、47例汉族)和30名正常人血清IL-6活性。 结果发现:维吾尔族与汉族MM患者间血清IL-6水平存在显著差异(P<0.01),MM患者血清IL-6水平明显高于正常人组(P<0.01),Ⅱ、Ⅲ期MM患者血清IL-6水平明显高于I期患者(P<0.05)。 结论: 维吾尔族与汉族MM患者血清IL-6水平存在显著差异性。血清IL-6水平可以作为判断MM患者病情严重程度、预后的指标之一,也是观察疗效、指导临床用药的综合多参数之一。

Detection of Serum Interleukin-6 Level in Patients with Multiple Myeloma of Vighur Nationality and Han Nationality and Its Clinical Significance
AbstractTo investigate the difference of serum interleukin-6 level in the patients with multiple myeloma (MM) of Vighur nationality and Han nationality and its clinical significance, the activity of serum interleukin-6 was detected in 80 patients with MM (33 Vighur patients, 47 Han patients) and 30 normal subjects as control by using sandwich ELISA with double antibody. The results showed that there was significant difference in serum interleukin-6 level between Vighur patients and Han patients(P<0.01). The serum interleukin-6 level of patients with multiple myeloma was significantly higher than that of nomal control (P<0.01). The serum interleukin-6 level of MM patients in second and third stage were also signifivantly higher than that of patients in first stage (P<0.05). It is concluded that there was significant difference in the serum interleukin-6 level between Vighur patients and Han patients. The serum interleukin-6 level can be considered as one of indexes to judge the patient severity and the prognosis, also as one of comprehensive multiparameters to evaluate the curative efficiency and guide the clinical application of drugs.
多发性骨髓瘤(MM)是造血系统常见的恶性肿瘤之一。白介素6(IL-6)是骨髓瘤细胞的主要生长因子[1],在多发性骨髓瘤的发生与发展中起着重要的作用。为此我们对我院近几年来维吾尔族和汉族80例MM患者及正常维吾尔族和汉族血清IL-6水平,采用双抗体夹心ELISA技术进行检测,发现维吾尔族和汉族MM患者的IL-6的水平存在明显不同,并且均高于正常对照组。现将结果报告如下。
材料和方法
病例
MM患者80例,其中维吾尔族33例,汉族47例。在33例维吾尔族患者中男性26例,女性7例,平均年龄54岁(41-68岁);47例汉族患者中男性24例,女性 23 例, 平均年龄69 岁 (56-82岁)。 随机选择来院体检的正常人维吾尔族15名,汉族15名,年龄(50-72岁)平均62岁。所有病例均符合MM诊断标准[2]。按Durie分期,其中I期28例,Ⅱ期22例,Ⅲ期30例。
检测仪器
IL-6 ELISA体外诊断试剂盒购置于北京晶美生物工程有限公司,DENLEY MK4型酶标仪为芬兰产品。
检测方法
对所有检测对象空腹采集静脉血,快速分离血清,于-20℃冻存。测定方法如下:①标准品:将标准品(1 000 pg/ml)进行稀释,按500、250、125、62.5、31.25、15.625、7.8125、0 pg/ml浓度建立标准曲线;②将包被IL-6单克隆抗体的酶标板取出并室温复温,除空白孔外,分别将标本或不同浓度标准品(100 μl/well)加入相应孔中,封板,37℃孵育90分钟;③洗涤4次;④除空白孔外,加入生物化抗体工作液100 μl/well,封板,37℃孵育30分钟;⑤洗涤4次;⑥除空白孔外,加入酶结合物工作液100 μl/well,封板,37℃孵育30分钟;⑦洗涤4次;⑧加入显色剂100 μl/well,37℃避光,孵育20分钟;⑨加入终止液100 μl/well,混匀后5分钟内测量OD值(450 nm);⑩依据标准曲线计算各待检标本IL-6含量(pg/ml)。
数据处理
实验数据以X±SD表示。采用华西医科大学PEMS 3.1统计软件包进行方差分析和t检验。
