【摘要】 目的 探讨进展期前列腺癌(pca)患者内分泌 治疗 前后前列腺特异性抗原(psa)变化的临床意义。 方法 测定pca患者内分泌治疗前和治疗后3、6、12、24个月血清psa。结果 治疗后3个月与治疗前相比血清psa下降明显。治疗后6个月达到psa谷值。结论 血清psa可作为判断pca内分泌治疗效果的标准，血清psa再次升高（＞4ng/ml）提示肿瘤 发展 为非激素依赖性前列腺癌，是预测生存率的一个独立因素。

the significance of the changing of serum psa after endocrinotherapy in advanced prostatic cancer
【abstract】 objective to detect the changing of the serum psa in patients with advanced prostatic cancer，and to discuss its significance.methods a total of 86 consecutive advanced prostate cancer patients treated with antiandrogen medications 24 months after castration were enrolled.serum levels of prostatic specific antigen(psa) were obtained just prior to castration，and 3，6，12，24 months after castration.results 3 months after treatment the serum psa reduced obviously than it before treatment.6 months after treatment the serum psa value was in the lowest.conclusion the serum psa can be a standard which judge the effect of endocrinotherapy in pca，and be a very useful prognostic marker.psa>4ng/ml showed the disease had developed to the hormone refractory pca.

2002年2月～2004年6月，检测中晚期前列腺癌(pca)患者治疗前后血清前列腺特异性抗原(psa)的动态变化，并对pca进展状况和生存率进行 分析 ，现将结果报告如下。
1 资料与方法
1.1 一般资料 本组患者86例，年龄58～94岁，平均75岁。均经前列腺穿刺活检或turp术后病理检查证实。术前病理报告gleason评分5～10分，平均7.2分，其中5分者8例，6分者18例，7分者28例，8分者25例，9分以上者7例。临床分期：c期28例，d1期33例，d2期25例（其中19例骨转移，4例肺转移，2例脑转移）。死亡6人。
1.2 治疗方法 86例中晚期pca患者均进行双侧睾丸切除加抗雄激素制剂（氟他胺）治疗。骨转移以及以后发展为非激素依赖性前列腺癌的患者给予放疗、化疗等治疗。
1.3 随访 检测术前、术后3、6、12、18、24个月血清psa值，同时检测服药期肝功能变化，psa值大于10ng/ml时做骨扫描检查。
1.4 统计学方法 所有数据用均数±标准差（x±s）表示，采用spss 8.0软件进行t检验，p<0.05差异有显著性。
2 结果
治疗前后血清psa检测结果，见表1。
