discussion of the relevant factors about the early diagnosis of atypical kawasaki disease
［abstract］ objective to explore the early diagnosis of atypical kawasaki disease（kd）.methods sum up the 38 cases in hospital of atypical kawasaki disease from september 1996 to september 2006，and analyse the early manifestations and laboratory targets of kawasaki disease retrospectively.results more than 80% of patients with kawasaki disease had white blood cells going up，the increases of erythrocyte sedimentation rate，the descent of hemoglobin，c-response protein going up，the proportion of albumin increasing and the increase of platelet.in the cases of this research，the original time that the above targets were appeared was 2nd day to 5th day in course of disease.conclusion if there are lack of typical symptoms of kawasaki disease，white blood cells going up，erythrocyte sedimentation rate increasing，hemoglobin descending，c-response protein going up，the proportion of albumin increasing and platelet increasing could used in the early diagnosis of atypical kawasaki disease（kd）.

皮肤黏膜淋巴结综合征是一种以发热、黏膜炎、皮疹、淋巴结肿大和指端皮肤改变为特征的急性全身血管炎症性疾病，本病于1967年由日本川崎首先报道，故又称为川崎病（kawasaki disease，kd）。如未及时诊断和治疗，可导致多脏器损害，尤其是冠状动脉的损伤，如冠状动脉炎、冠状动脉扩张和冠状动脉瘤，因此，必须引起临床医师高度重视。由于本病的诊断主要依据临床表现而缺乏特异性的实验室诊断指标，给早期诊断造成一定困难。为了总结临床经验，争取对kd进行早期诊断及治疗，现将我院1996年9月～2006年9月收住院的38例不典型川崎病病例分析报告如下。
1 资料与方法
1.1 一般资料 38例kd系我科1996年9月～2006年9月住院患儿，男26例，女12例；年龄8个月～12岁，其中＜3岁25例，＞3岁13例。38例患儿初次诊断的疾病：上呼吸道感染及结膜炎15例，肺炎5例，败血症3例，颈淋巴结炎3例，药疹3例，尿路感染2例，肾炎2例，其中1例以急性肾功能衰竭起病，传染性单核细胞增多症、猩红热、风湿热、麻疹各1例。
1.2 治疗方法 通过动态观察，追踪发病经过，且排除其他类似疾病，最后根据不典型川崎病的诊断标准［1］确诊，延迟确诊时间7～12天。确诊后38例均给予大剂量免疫球蛋白（high dose immunity globu bin，hdig）2 g/kg体重，32例应用1剂后体温在24～36 h下降；6例应用2剂后体温在72 h内降至正常。38例均口服肠溶阿司匹林治疗，均临床治愈。
1.3 观察方法 观察方法包括：（1）分析发热5天内出现临床症状的时间及脏器受损的情况。（2）根据文献报道［2～4］kd的实验室检查:早期阳性率较高的项目有wbc升高、血沉增快、血红蛋白下降、c反应蛋白升高、血浆白蛋白降低、血小板增高等，对kd早期诊断有意义。笔者选择以上指标对我院38例不典型kd以上实验室检查进行分析。
2 结果
2.1 临床表现 除发热以外的5项临床表现中，本组患儿多数早期具备2～3项，临床表现见表1。

2.1.1 早期临床表现 肢端早期红肿14例；有一过性皮疹31例，为斑丘疹，有4例皮疹反复出现；早期有双眼结膜明显充血28例；口腔黏膜充血及杨梅舌，口唇发红、皲裂干燥16例；颈部淋巴结肿大27例。
2.1.2 其他系统受累情况 心脏17例，表现为心电图及心脏彩超异常；肝脏4例，表现为肝功能异常及肝脏肿大；肺脏5例，表现为咳嗽、气促及x线胸片改变；肾脏4例，表现为血尿、蛋白尿及肾功能损害，其中1例以急性肾功能衰竭为首发表现［5］；消化系统5例，表现为腹痛、腹泻。
