【摘要】 目的 探讨阿立哌唑治疗儿童精神分裂症的疗效及安全性。 方法 将50例儿童精神分裂症患者随机分为两组各25例，研究组给予阿立哌唑治疗，对照组给予利培酮治疗，观察8 w。于治疗前及治疗1 w、2 w、4 w、6 w、8w末采用潘氏量表评定临床疗效，副反应量表及锥体外系反应量表评定不良反应。 结果 治疗8 w末研究组总有效率84.0%，对照组92.0%，两组总体疗效相当（χ2=0.271，P>0.05）。潘氏量表评分两组治疗2w末起均较治疗前有极显著性下降(P均<0.01)；研究组治疗1 w末潘氏量表P分较对照组下降显著（P<0.05）。研究组肌强直、震颤、静坐不能、体重增加不良反应发生率显著低于对照组（P<0.05）。 结论 阿立哌唑治疗儿童精神分裂症疗效与利培酮相当，缓解阳性症状更快，安全性高，依从性好。

A control study of aripiprazole in the treatment of childhood schizophrenia
【Abstract】 Objective To explore the curative effect and safety of aripirazole in the treatment of childhood schizophrenia．Methods 50 patients with childhood schizophrenia were randomly divided into research(took aripirazole, n=25) and control group( did risperidone, n=25) for 8 weeks. Before treatment and at the ends of the 1st, 2nd, 4th, 6th and 8th week, curative effects were assessed with the Positive and Negative Syndrome Scale(PANSS) and adverse effects with the Treatment Emergent Symptom Scale and the Scale for Extrapyramidal Side Effects(ESRS), respectively. Results At the end of 8th week, total effective rates were 87.5％ in the research and 92％ in the control group respectively, the total effectiveness of the 2 groups was equivalent(χ2 = 0.271,P>0.05)．Since the end of the 2nd week, the PANSS scores of both groups decreased very significantly compared with pretreatment(both P<0.01); at the end the 1st week, P score of the PANSS in the research group decreased more significantly than in the control group(P<0.05). Conclusion Aripiprazo1e has therapeutic equivalence to risperidone in childhood schizophrenia, relieves positive symptoms faster and has higher safety and better compliance.

由于儿童正处于生长发育和学习阶段，当治疗儿童精神分裂症时要考虑其个体的特殊性。有报道［1］表明，利培酮治疗儿童精神分裂症疗效显著、安全性高。为此，作者应用阿立哌唑与其进行了临床治疗对照研究，以探讨阿立哌唑治疗儿童精神分裂症的疗效及安全性，现将结果报告如下。
1 对象与方法
1.1 对象 样本选自2004年12月～2005年12月在我院住院的儿童精神分裂症患者。入组标准：（1）符合《中国精神障碍分类与诊断标准》第3版(CCMD3)精神分裂症诊断标准［1］；（2）潘氏量表（PANSS）总分≥60分，年龄9 a～14 a，性别不限；（3）入院前未使用抗精神病药物治疗；（4）排除脑器质性疾病、精神发育迟滞及其他躯体疾病。符合上述入组条件者共50例，随机分为两组各25例。研究组男11例，女14例；平均年龄（10.07±2.35） a，平均病程（1.64±3.74） mo，平均住院时间（2.30±1.74） mo。对照组男10例，女15例；平均年龄（9.37±4.08） a，平均病程（1.87±2.96） mo，平均住院时间（1.90±2.48） mo。两组间一般资料比较差异均无显著性(P均>0.05)。
1.2 方法
1.2.1 给药方法 研究组口服阿立哌唑5 mg～15 mg・d-1治疗，最大剂量20 mg・d-1。对照组口服利培酮1 mg～4 mg・d-1。两组治疗剂量均根据患者的不良反应及病情变化调整，1 w内加至治疗剂量，疗程8 w。治疗中可酌情使用苯二氮艹卓类药物和抗胆碱能药。
1.2.2 疗效评定 于治疗前及治疗1 w、2 w、4 w、6 w、8 w末采用PANSS评定临床疗效，副反应量表（TESS）评定不良反应，锥体外系副反应量表（ESRS）评定锥外系反应。依据PANSS总分减分率判定临床疗效，减分率≥75％为痊愈，≥50％为显著进步，≥25％为好转，<25％为无效。
1.2.3 实验室检查 治疗前检查心电图、脑电图、肝功、血常规、尿常规及测体重，治疗后除脑电图外其余各项检查1次・w-1。
1.3 统计方法 所有数据应用SPSS10.0统计软件处理，并进行t检验、χ2检验。
